Public health units maintain immunization records for children who attend licensed child care, elementary, and secondary school in Ontario using the provincial database (Panorama). Reporting immunizations helps protect children and the community from vaccine-preventable diseases.

Please report to Grand Erie Public Health each time your child is immunized. Parents and guardians are responsible for keeping records up to date under the Immunization of School Pupils Act (ISPA) and the Child Care and Early Years Act (CCEYA).

Remember: Physicians are not required to report immunizations to public health. Please report every time your child receives a vaccine.

  • GEPH maintains and assesses records for child care and school‑aged children only.
  • We only have access to records that have been reported to a public health unit in Ontario.
  • Community physicians do not have direct access to Panorama to report immunizations.

  • Report online here (Note: use the Ontario Immunization Identification Number or Ontario Health Card Number)

  • In person: 194 Terrace Hill St., Brantford
    • Weekdays from 8:30 a.m. – 4:30 p.m.
    • There is an after hours dropbox located outside the main entrance

Public health units retain student immunization records (reported to public health) and are required to keep them for 10 years. Your record reflects what has been reported to a public health unit in Ontario.

Note: for individuals 18 and older, GEPH requires permission from the individual to release their vaccination records as requested. This is in accordance with The Personal Health Information Protection Act.

What you will need

  • Your child’s immunization record
  • Ontario Health Card Number and Stock Control Number (9‑character code on back)
  • Child’s first and last name, sex, date of birth, and postal code

Note: If the details above don’t match the provincial record, you may need your child’s unique Ontario Immunization ID. If you received a letter from public health, the Immunization ID appears under the address.

Steps

  1. Click on “How to report your child's immunizations” above.
  2. Choose Public device or Personal device.
  3. Read the Acceptable Use Policy and select I accept.
  4. Under Verify with Health Card Number, enter the requested information and select Verify patient.
  5. Set a 6‑digit PIN (or enter your existing PIN) and continue.
  6. Review your child’s record. Select Submit immunizations to add any missing vaccines.
  7. Upload a clear photo or scan of the record to help with verification. Double‑check dates. Common error: mixing up day and month when dates are numeric.

Submitted immunizations will not appear instantly. Please allow up to five business days for processing (up to two weeks during peak periods).
