Brantford, Ont. – Grand Erie Public Health (GEPH) is continuing with an infection prevention and control lapse investigation regarding a dental office, Dr. George Chan Dentistry, formerly operating at 353 St. Paul Ave. in Brantford. GEPH wants to remind former dental office patients who had dental procedures between November 3, 2021, and November 3, 2025, to discuss testing for blood-borne infections (hepatitis B, hepatitis C, HIV) with their primary healthcare provider if they have not already done so.
Following the identification of the lapse and recommendations made in consultation with infection prevention and control (IPAC) experts from Public Health Ontario (PHO), GEPH initiated a patient recall and testing recommendation process.
Former patients of Dr. George Chan Dentistry seen between November 3, 2021, and November 3, 2025, were mailed a notification letter earlier this year with advice to speak with their healthcare provider about recommended blood-borne infection testing. To date, approximately half of those who were mailed a notification letter have completed the recommended testing. Former patients who have not yet been tested are encouraged to do so, as the information gathered through testing remains an important part of the ongoing public health investigation. At this time, no cases of blood-borne infection have been confirmed to be linked to the dental clinic.
Grand Erie Public Health appreciates the former patients who have already completed the recommended testing. Former patients who received a notification letter and have not yet completed the recommended testing are encouraged to:
- Consult their primary care provider or visit a walk-in clinic to discuss testing for blood-borne infections.
- Visit www.geph.ca/recall to complete a questionnaire and print off a testing requisition. This requisition form is required. If you do not have access to a printer, please ask your provider’s office to print a copy.
- If you don’t have a primary care provider, you can access care at the Primary Care Access Clinic by calling 226-388-4725 or by booking an appointment with the Virtual Urgent Care Clinic at www.urgentcareontario.ca.
- Do not visit an emergency department solely for testing, unless medically necessary.
While the overall risk of infection remains low and no additional public health recommendations have been made at this time, testing is an important part of the ongoing investigation.
GEPH conducts infection prevention and control (IPAC) inspections of regulated health professionals, including dental clinics, on a complaint basis. The Royal College of Dental Surgeons of Ontario, which regulates dental practice in Ontario, was notified of this investigation. The dental practice voluntarily permanently closed in November 2025.
